I am sorry we do not have their log form in our system. You can send me 2 -3 of your log forms with the company name and address preprinted on them and I can add it to your account.
Please DO NOT fold the logs when you send them in, it messes up the scanning. Also include a note with your name and phone number so I know which account to add it to.
