I apologize, I do not have their log form. If your company gives you the original (not copies) preprinted logs with the company name and address on the log then you can send me 2 - 3 log pages. Please DO NOT fold the logs when you send them in. Also please include a note with your name and phone number so I know which account to add it to. Depending on the log form it can take up to a week to digitize the log. I will give you a call when I have finished the log form and added it to your account.
