Due to copyright/licensing issues we can't offer forms for carriers unless you work for that carrier... the company name and address are locked down on those forms.
I know that we are going to offer a new city list for Logbook GPS that has fewer silly points (even though those same silly points came from a government-certified list of geographic places!)
