I don't mean to be disrespectful, but you are out of line. You posted your message one week ago, and I replied at 5:00 in the morning to your post from 9:40 the previous night. See the message in the sales you titled "Issue with Logbook" (I believe it's the one just prior to this one).

I'll repeat myself here...
If your form is preprinted with carrier name and address, you can use that form. We will need you to send us unused paper copies so we can make a template. If the form is blank, then we need permission from the copyright holder before we can make a template. It may be easier to use one of our other standard forms which we will be glad to attach to your account.
