    Start Point and Destination

    I don't see a start point or destination field on the form. Don't you have to have these field for the DOT

    You have to provide location at each change-of-duty. If you click on the grid to note a change-of-duty, you'll see all the fields below it.

    If you are talking about the log's overall start/end locations (often shown at the bottom of the form, not in the remarks section on the log grid), those used to be required many years ago but have been deleted as requirements by the FMCSA.
